Who Provides Care?

 

Our team consists of highly qualified and experienced professionals that will address the physical, emotional and spiritual needs of our patients and their loved ones. 

​

 

Medical Director/ Hospice Physician who works in conjunction with the patient’s Primary Physician.

 

 

Hospice Skilled Nurses (RN and LVN), assesses and provides pain management and symptom control and maintains communication and coordination with entire IDT. Available 24 hours a day, 7 days a week.

 

 

Certified Hospice Aides provides and assists patients with their personal care. 

 

 

Medical Social Worker provides psychosocial and emotional support to our patients and their loved ones.

 

 

Chaplain/ Spiritual Counselors provides spiritual and bereavement support for patient and family members.

 

 

Hospice Volunteers are trained to provide respite, emotional support, companionship, and assist with day-to-day activity.
